These Terms and Conditions govern your use of our services, website, and content. By using our services, you agree to these terms.

  • Illuminate Health offers both in-person and virtual services. In-person services take place at Apple Fitness and Health in Cookeville, TN.

  • You must be at least 18 years old to schedule services.

    Minors (under 18) may receive treatment, but a parent or legal guardian must provide consent at the first visit and be present if required.

  • We collect your payment method at your initial consultation and charge after each appointment.

    Payments must be made using a valid credit or debit card.

    We do not accept insurance.

  • Appointments must be canceled or rescheduled at least 24 hours in advance to avoid being charged the full session fee.

    No-shows or late cancellations will result in the full fee being charged. This is evaluated on a case by case basis.

  • Illuminate Health provides chiropractic care and rehab coaching, but we do not replace medical diagnosis or emergency care. If you have a medical emergency, call 911 or visit the nearest hospital.

    Virtual services have limitations, and some conditions may require in-person evaluation.

    Results from rehab and chiropractic care vary based on individual factors. No guarantees are made regarding outcomes.

    You assume full responsibility for any risks associated with exercises, movement strategies, or treatments provided.

  • You agree to provide accurate medical history and information.

    You understand that results vary and that success depends on adherence to rehab and treatment plans.

    You agree not to hold Illuminate Health liable for any personal outcomes related to your participation in our services.

  • To the fullest extent permitted by law, Illuminate Health shall not be liable for any direct, indirect, incidental, or consequential damages, injuries, or losses arising from your use of our services.

  • All content, logos, branding, and materials provided by Illuminate Health are our intellectual property and may not be copied, distributed, or modified without our permission.

  • Your personal and health information is handled in accordance with our Privacy Policy.

  • We may update these Terms and Conditions at any time. Continued use of our services after changes indicates your acceptance of the new terms.
